Position Summary:

The Team Manager is responsible for the direct supervision and clinical oversight of hospice staff, ensuring the delivery of high-quality, compassionate care. This role also serves as a key clinical resource and support for team members, promoting excellence in patient outcomes and staff development.

Responsibilities

Responsibilities of Position:

  • Assist with the development and implementation of Agency policies and procedures.
  • Manage departmental budget and control expenditures.
  • Enforce departmental vision, goals, and implementation plan.
  • Build and maintain an effective team.
  • Participate in the Agency survey. (ie. Ohio, Joint Commission, Finance, etc.
  • Provide expert leadership regarding optimum symptom management and overall care provided.
  • Collaborate with Medical Director, primary physicians, and/or consulting physicians regarding appropriateness for re-admission or re-certification and regarding appropriate symptom management.
  • Participate in Interdisciplinary Team Conferences.
  • Participate in the development, review, and evaluation of the patient’s Plan of Care across all levels and settings of routine hospice care, including crisis care, end-of-life care, and GIP through ongoing review of clinical records and chart reviews.
  • Monitor quality of care and appropriateness of care provided in all settings, whether that is in the patient’s home, skilled nursing, or assisted living setting.
  • Monitor clinical documentation for timeliness, appropriateness, and compliance with Agency policy, accreditation bodies, and governing requirements.
  • Assist the Director of Nursing with additional documentation requests from Payer Service.
  • Evaluate staff productivity. Make supervisory visits to evaluate staff competency annually.
  • Participate in visits as needed.
  • Work evenings and weekends as an administrative assistant on call.
  • Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.
  • Other duties as assigned.
Qualifications

Education and Experience:

  • Graduation from an approved School of Nursing.
  • Current RN licensure in the State of Ohio.
  • Minimum of three years of experience in Medical/Surgical, Oncology or community health nursing.
  • Hospice experience preferred.
  • Minimum of one year of management experience.

Special Skills and Requirements:

  • Shall have a valid driver’s license.
  • Excellent customer service skills.
  • Detail-oriented with excellent communication, documentation, and organizational skills.
  • Ability to follow instructions.
  • Ability to work independently without supervision.
  • Travel to other sites as needed. Travel may be in inclement weather.
  • Ability to lift 25 lbs of supplies or equipment.
  • Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking.
