HomeFirst Home Care, LLC is seeking a qualified Registered Nurse (RN) Supervisor to direct, manage, and oversee personal care services in full compliance with the Virginia Department of Health (VDH) Regulations for the Licensure of Home Care Organizations (12VAC5-381). The RN Supervisor ensures high-quality, person-centered care, conducts client assessments, develops individualized Plans of Care, evaluates caregiver competencies, and conducts mandatory quarterly supervisory field visits.

• Conduct initial face-to-face in-home assessments for all new personal care clients.

• Develop written Plans of Care detailing client functional limitations, allowed activities, dietary needs, specific personal care tasks, and service frequency.

• Re-evaluate and update client Plans of Care whenever there is a significant change in client condition or at required regulatory intervals.

• Perform and document face-to-face supervisory visits of assigned clients and personal care attendants at least once every 90 days (or more frequently based on client acuity).

• Evaluate caregiver clinical competencies during initial hiring and annually thereafter.

• Ensure caregivers complete a minimum of 12 hours of annual in-service training and education as mandated by VDH.

• Review all caregiver service documentation and clinical logs for accuracy, completeness, and adherence to established care plans.

• Report changes in client condition, potential safety hazards, or suspected abuse/neglect to the Agency Administrator immediately.

• Participate in VDH on-site survey inspections and clinical audit readiness.

• Licensure: Current, active, and unencumbered Registered Nurse (RN) license issued by the Virginia Board of Nursing or a recognized Nurse Licensure Compact (NLC) multistate privilege.

• Experience: Minimum of 2 years of clinical nursing experience; at least 1 year of experience in home health, personal care home care, or community health nursing strongly preferred.

• Must successfully pass a Virginia State Police criminal background check clearing all state-mandated barrier crimes under Virginia Code § 32.1-162.9.

• Valid Virginia driver’s license, reliable personal vehicle with active automobile insurance.
