- Coordinate and manage a caseload of hospice patients, ensuring each receives personalized care tailored to their unique needs.
- Conduct regular assessments to monitor patients' conditions and adjust care plans as necessary, always prioritizing their comfort and dignity.
- Collaborate with interdisciplinary team members to develop comprehensive care strategies that address physical, emotional, and spiritual aspects of patient well-being.
- Provide education and support to patients and their families, empowering them with the knowledge and resources needed to navigate the hospice journey.
- Document patient care activities accurately and promptly, maintaining clear communication with the team to ensure continuity of care.
- Serve as a compassionate advocate for patients, ensuring their voices are heard and their preferences respected in all care decisions.
- Facilitate smooth transitions between care settings, coordinating with external providers to maintain seamless patient experiences and outcomes.
- Experience in hospice or palliative care nursing, with a deep understanding of the unique needs of patients and families during end-of-life care.
- Ability to manage a diverse caseload effectively, ensuring personalized and compassionate care for each patient.
- Proven track record of collaborating with interdisciplinary teams to develop holistic care plans that address physical, emotional, and spiritual needs.
- Strong communication skills to educate and support patients and families, empowering them with knowledge and resources.
- Proficiency in documenting patient care activities accurately and promptly, ensuring clear communication with the care team.
- Ability to advocate for patients' preferences and ensure their voices are heard in all care decisions.
- Experience in facilitating transitions between care settings, coordinating with external providers for seamless patient experiences.
Compensation
$53 - $56 hourly
