Responsibilities
- Coordinate and manage comprehensive care plans for hospice patients, ensuring their unique needs are met with empathy and precision.
- Conduct regular home visits to assess patient conditions, providing timely interventions and adjustments to care plans as needed.
- Collaborate closely with interdisciplinary team members to deliver holistic, patient-centered care that honors each individual's journey.
- Educate patients and their families about the hospice process, fostering understanding and comfort during challenging times.
- Monitor and manage symptoms effectively, ensuring patients experience the highest possible quality of life.
- Document patient progress and care activities accurately, maintaining clear and thorough records for seamless team communication.
- Advocate for patients' needs and preferences, ensuring their voices are heard and respected in all care decisions.
Qualifications
- Current Registered Nurse (RN) license in the state of New Jersey.
- Experience in hospice or palliative care, demonstrating a compassionate approach to patient management.
- Ability to develop and implement comprehensive care plans tailored to individual patient needs.
- Proven track record of collaborating effectively with interdisciplinary teams to enhance patient care.
- Strong communication skills to educate and support patients and families, fostering confidence and understanding.
- Proficiency in administering medications and treatments, prioritizing patient comfort and safety.
- Excellent documentation skills to maintain accurate and timely patient records, ensuring clear communication with the care team.
Compensation
$53 - $56/hour
