- Deliver compassionate, patient-centered care by managing acute and chronic pain in skilled nursing and assisted living facilities throughout the Twin Cities metro area.
- Collaborate with primary care providers and interdisciplinary teams to develop comprehensive pain management plans.
- Conduct thorough patient assessments to identify pain sources and tailor individualized treatment strategies.
- Utilize evidence-based practices to prescribe medications responsibly, ensuring safe and effective pain relief.
- Educate patients and their families on pain management techniques, fostering understanding and adherence to treatment plans.
- Monitor and evaluate patient progress, adjusting care plans as necessary to optimize outcomes and enhance quality of life.
- Implement risk assessment tools and utilize prescription drug monitoring databases to mitigate opioid-related issues proactively.
- At least 1-2 years as a Nurse Practitioner (NP) or Physician Assistant (PA).
- Master's degree or higher.
- Experience working in a skilled nursing facility (SNF) strongly preferred.
- Pain management experience strongly preferred but not required.
- Ability to build and foster trust-based relationships with patients, families, and diverse healthcare teams.
- Experience reviewing and maintaining up-to-date medical records in EHR systems.
- Valid driver's license and auto insurance.
- Reliable transportation to travel between care facilities in and around the Twin Cities metro (Minneapolis, St. Paul, and the surrounding suburbs).
Compensation
$100,000 - $150,000 yearly
