Our busy home health agency is seeking an experienced home health aide to assist our case managers in implementing patient-specific care plans. You’ll give our patients companionship and help with basic medical care, light housekeeping duties, personal care, and hygiene tasks so that they are empowered to live happily at home. • Provide compassionate companionship and emotional support to patients, ensuring they feel valued and understood.
• Monitor and report changes in patients' health status to the healthcare team, fostering proactive and responsive care.
