We are seeking a compassionate, dependable, and skilled Registered Nurse (RN) to join our growing home health team. In this role, you will provide high-quality nursing care to patients in the comfort of their homes while promoting independence, safety, and improved health outcomes. You will collaborate with physicians, therapists, caregivers, and other healthcare professionals to implement individualized care plans and ensure patients receive exceptional care.

• Provide skilled nursing care in accordance with physician orders and care plans
• Perform patient assessments and ongoing evaluations
• Administer medications, treatments, and wound care as prescribed
• Monitor and document patient progress accurately and in a timely manner
• Educate patients and families on disease management and care techniques
• Coordinate care with interdisciplinary team members
• Ensure compliance with all state, federal, and agency regulations

• Current, active RN or LPN license in the state of practice
• Minimum of [1 year] nursing experience (home care experience preferred)
• Strong clinical and assessment skills
• Excellent communication and organizational abilities
