The Family Nurse Practitioner or Physician Assistant will provide comprehensive outpatient primary care for adult patients, as well as evaluate and manage patients with pulmonary and occasional sleep-related conditions.
Primary care responsibilities will include preventive care, routine health maintenance, chronic disease management, acute and same-day visits, medication management, health education, screening, and care coordination.
The FNP/PA will also evaluate and manage common pulmonary and sleep conditions in collaboration with the supervising physician and clinical team. The position offers an opportunity for an advanced practice provider who enjoys variety and would like to practice both primary care and specialty medicine in a growing outpatient setting.
As one of our founding clinical team members, you will play an important role in shaping patient care, establishing clinical workflows, and helping develop both our primary care and specialty services. We are seeking a motivated provider with a strong foundation in primary care and an interest in pulmonary and sleep medicine. Pulmonary experience is highly desirable but not required, as we are willing to provide training and mentorship.
Job Type:
Part-Time or PRN (Hourly), with the opportunity to transition to Full-Time as the practice grows.
Primary Care Clinical Responsibilities
Provide comprehensive primary care to adult patients across a broad range of health conditions.
Perform new patient and established patient evaluations, including comprehensive histories and physical examinations.
Diagnose, treat, and manage common acute and chronic medical conditions within scope of practice.
Provide preventive healthcare and routine health maintenance.
Perform age- and risk-appropriate health screenings and recommend appropriate follow-up.
Manage chronic conditions such as hypertension, hyperlipidemia, diabetes, obesity, thyroid disorders, GERD, cardiovascular risk factors, and other common adult medical conditions.
Evaluate and treat common acute illnesses, including upper respiratory infections, bronchitis, sinusitis, urinary tract infections, minor infections, and other appropriate outpatient conditions.
Review and manage routine laboratory and diagnostic testing.
Order appropriate laboratory studies, imaging, and diagnostic testing within scope of practice and clinic protocols.
Prescribe and manage medications, including refills and medication adjustments, in accordance with applicable state law, credentialing requirements, and clinic policies.
Provide preventive counseling regarding nutrition, exercise, weight management, smoking cessation, alcohol use, vaccinations, and healthy lifestyle choices.
Provide women's and men's health services appropriate to the provider's training, scope of practice, and clinic services.
Evaluate and manage obesity and related metabolic and cardiovascular risk factors.
Provide appropriate follow-up and monitoring of abnormal laboratory, imaging, and screening results.
Coordinate care with specialists, hospitals, pharmacies, laboratories, imaging centers, and other healthcare providers.
Obtain and review outside medical records, specialist notes, hospital records, laboratory results, and diagnostic studies.
Provide patient education regarding diagnoses, treatment plans, medications, preventive care, and lifestyle modifications.
Recognize urgent or complex medical conditions and coordinate timely evaluation or referral when a higher level of care is needed.
Participate in establishing and improving primary care workflows, protocols, and clinical processes as the practice grows.
Pulmonary & Sleep Medicine Clinical Responsibilities
Evaluate and manage adult patients with pulmonary and occasionally sleep-related conditions.
Obtain comprehensive medical histories, including past medical and surgical history, with special attention to pulmonary pathologies.
Collect tobacco and vaping history, occupational exposures, prior hospitalizations, current oxygen therapy, and CPAP/BiPAP use.
Assess patients presenting with respiratory symptoms such as cough, wheezing, shortness of breath, chest tightness, or other respiratory complaints.
Evaluate sleep-related symptoms including fatigue, snoring, excessive daytime sleepiness, witnessed apneas, and other symptoms concerning for sleep apnea.
Review referrals, specialist and primary care notes, laboratory studies, imaging reports, pulmonary diagnostics, sleep studies, pulmonary function tests, CPAP/BiPAP compliance reports, hospital records, and other relevant documentation.
Order appropriate laboratory testing and imaging to support the evaluation and management of suspected pulmonary or sleep-related disease within scope of practice and clinic protocols.
Educate patients regarding disease processes and treatment plans, including inhaler technique, nebulizer use, asthma and COPD management, smoking cessation, sleep apnea testing, CPAP/BiPAP therapy, weight management, respiratory infection prevention, vaccinations, and chronic disease management.
Prescribe medications and inhalers and coordinate DME needs, including oxygen, ventilators, CPAP/BiPAP equipment, and related supplies, in accordance with applicable state law, credentialing requirements, and clinic policies.
Provide follow-up care for conditions including asthma, emphysema, chronic bronchitis, pneumonia, interstitial or restrictive lung disease, chronic cough, sleep apnea, chronic hypoxia, post-COVID respiratory conditions, obesity hypoventilation syndrome, and other respiratory conditions.
Collaborate with the supervising physician regarding abnormal pulmonary testing, imaging findings, complex cases, and atypical respiratory conditions.
Support prior authorization processes for medications, imaging studies, pulmonary testing, sleep studies, PAP therapy, oxygen therapy, and DME.
Recognize urgent or complex pulmonary findings and collaborate with the physician to ensure timely intervention or referral.
Coordinate care with referring providers, specialists, hospitals, imaging centers, sleep laboratories, pharmacies, pulmonary function laboratories, and DME providers.
Required
Current, unrestricted Texas RN/APRN license for Nurse Practitioner candidates or Texas Physician Assistant license for PA candidates.
Current Family Nurse Practitioner (FNP-C or FNP-BC) certification for NP candidates or NCCPA certification for PA candidates.
Prescriptive authority or eligibility to obtain prescriptive authority through credentialing, as applicable to the provider's license.
Current BLS certification.
Ability to pass a background check and meet all clinic onboarding requirements.
Preferred
Two to five years of clinical experience in primary care, pulmonology, sleep medicine, or another outpatient specialty.
Experience managing common adult primary care conditions, including hypertension, diabetes, hyperlipidemia, obesity, thyroid disease, and other chronic medical conditions.
Experience providing preventive care and health maintenance.
Experience managing asthma, emphysema, bronchitis, pneumonia, interstitial or restrictive lung disease, chronic cough, sleep apnea, chronic hypoxia, post-COVID syndrome, obesity hypoventilation syndrome, and other respiratory diseases.
Experience reviewing pulmonary function tests, chest X-rays or CT scans, sleep studies, oxygen therapy documentation, and CPAP/BiPAP compliance reports.
Familiarity with basic EMR systems and outpatient primary care and specialty practice workflows.
Pulmonary or sleep medicine experience is highly desirable but not required; training and mentorship will be provided.
Reliable transportation and willingness to work at both the Austin and Leander clinic locations based on patient and clinic needs.
Professional, dependable, organized, and adaptable, with the ability to thrive in a growing practice.
Ability to work independently while collaborating closely with the supervising physician and clinical team.
Strong communication and interpersonal skills with a patient-centered approach to care.
Compensation
$55 - $75 hourly
We are launching a growing outpatient medical practice serving patients in the Austin, Leander, and surrounding communities. Our clinic provides comprehensive primary care along with pulmonary and sleep medicine services for adult patients. Our clinical team is led by a board-certified internist and pulmonologist dedicated to providing compassionate, evidence-based care.
